Objective: To assess the reproducibility of symptoms in drug challenge tests. Methods: The study included patients with positive cutaneous or challenge test throughout 2019. For each patient, clinical suspicion according to Karch-Lasagna algorithm was registered. Primary outcome was the reproducibility of symptoms in the provocation tests using a paired analysis of data with McNemar test. Results: Eighty-nine patients were included, 16 of them presented more than one positive test. Thirty were skin tests positive and 75 reacted to provocation tests. Eighty nine percent of patients who reacted in challenge test were probably or possibly reactors according to Karch-Lasagna scale. Symptoms of initial reaction did not differ from those triggered in challenge tests. Conclusions: Karch-Lasagna scale is useful in predicting the response to drug provocation tests. In most of the positive studies, results were suggested by clinical history and no differences were found between symptoms triggered in challenge test and that referred to in the previous reaction.


Objetivo: Evaluar la reproducibilidad de los síntomas en pruebas de exposición con fármacos. Métodos: Estudio retrospectivo, efectuado en pacientes con prueba cutánea o exposición positiva, atendidos en 2019. De cada paciente se registró la sospecha clínica según el algoritmo de Karch-Lasagna. El resultado principal fue la reproducibilidad de síntomas en las pruebas de exposición, mediante el análisis de datos emparejado con prueba de McNemar. Resultados: Se incluyeron 89 pacientes, y de estos 16 reportaron varias pruebas positivas. Se obtuvieron 30 pruebas cutáneas y 75 de exposición positivas. En el 89% de las pruebas de exposición positivas, las reacciones iniciales se clasificaron en probables o posibles, según la escala de Karch-Lasagna. Los síntomas reportados en la reacción inicial no difirieron de los de las pruebas de exposición. Conclusiones: La escala de Karch-Lasagna es un método útil para predecir la respuesta en las pruebas de exposición con fármacos. En la mayor parte de las pruebas positivas, los resultados fueron sugeridos por la historia clínica, sin diferencias entre la manifestación de síntomas en la prueba de exposición versus los referidos en la reacción inicial.

Background Primary eosinophilic gastrointestinal diseases (EGID) are chronic inflammatory disorders of the gastrointestinal tract with unknown etiology. Features, utility, and evolution are still unknown in screening for EGID in adult patients with eosinophilic esophagitis (EoE). Objectives To evaluate the prevalence, characteristics, comorbidities, and evolution of EGID in adults diagnosed with EoE and investigate differences between both groups. Methods Prospective unicenter observational and analytical study. Gastric and duodenal biopsies were obtained during upper baseline endoscopy in all consecutive EoE adult patients evaluated. A colonoscopy with colon biopsies was performed upon persistent diarrhea and normal duodenal biopsies. Results 212 EoE patients were included. Nine patients (4.3%) also showed significant eosinophilic infiltration in at least one organ within the digestive tract. The most common site affected was the small bowel (78%). Gastrointestinal symptoms (43% vs. 100%, p<0.002) and, more specifically, either abdominal pain or diarrhea (17% vs. 78%, p<0.001), some food sensitizations, and digestive comorbidities (p<0.05) were significantly more common in patients with EGID. Gastrointestinal symptoms were present in 94/212 (44%) patients, of whom 9 (10%) had EGID. Considering only abdominal pain or diarrhea, 20% suffered from it. Conclusions EGID rarely coexist with EoE, even when gastrointestinal symptoms are present. These findings advise against routine gastric, duodenal, or colon biopsies in adult EoE patients with gastrointestinal symptoms. Most of the characteristics of EoE do not change due to having EGID except gastrointestinal symptoms, digestive comorbidities, and sensitizations to some foods. The evolution was generally favorable despite intermittent adherence to treatment, especially maintenance (AU)


Antecedentes Las enfermedades gastrointestinales eosinofílicas primarias (EGIE) son trastornos inflamatorios crónicos del tracto gastrointestinal con etiología desconocida. Aún se desconocen las características, la utilidad y la evolución en el cribado de EGIE en pacientes adultos con esofagitis eosinofílica (EoE). Objetivos Evaluar la prevalencia, características, comorbilidades y evolución de las EGIE en adultos diagnosticados de EoE e investigar las diferencias entre ambos grupos. Métodos Estudio observacional y analítico prospectivo de un único centrro. Se obtuvieron biopsias gástricas y duodenales durante la endoscopia digestiva superior en todos los pacientes adultos con EoE consecutivos evaluados. Se realizó colonoscopia con biopsias de colon ante diarrea persistente y biopsias de duodeno normales. Resultados Se incluyeron 212 pacientes con EoE. Nueve pacientes (4,3%) también mostraron infiltración eosinofílica significativa en al menos un órgano dentro del tracto digestivo. El sitio más común afectado fue el intestino delgado (78%). Los síntomas gastrointestinales (43 vs. 100%, p < 0,002) y, más específicamente, dolor abdominal o diarrea (17 vs. 78%, p < 0,001), algunas sensibilizaciones alimentarias y comorbilidades digestivas (p < 0,05) fueron significativamente más comunes en pacientes con EGIE. Los síntomas gastrointestinales estuvieron presentes en 94/212 (44%) pacientes, de los cuales nueve (10%) tenían EGIE. Considerando solo dolor abdominal o diarrea, 20% la padecía. Conclusiones Las EGIE rara vez coexisten con EoE, incluso cuando hay síntomas gastrointestinales. Estos hallazgos desaconsejan las biopsias gástricas, duodenales o de colon de rutina en pacientes adultos con EoE con síntomas gastrointestinales. La mayoría de las características de la EoE no cambian por tener EGIE, excepto los síntomas gastrointestinales, las comorbilidades digestivas y las sensibilizaciones a algunos alimentos (AU)

Allergy to fruits and vegetables is the most common primary food allergy in Mediterranean countries, especially the lipid transfer proteins (LTPs) syndrome. This study is the first research that studies multiple clinical, allergological and therapeutics characteristics of allergies to plants in the South-East of Spain, and assessing whether these characteristics differ in emergency room or outpatient clinic. This is a prospective study of patients who consult for the first time for allergy to vegetables at Granada, Spain. We record demographic data, symptoms, allergological study and indicated therapy. The characteristics obtained agree with the current bibliographic except the higher prevalence than other areas of positive sensitization of Pru p 3, Cor a 9 and Ara h 9, which predisposes to severe allergic reactions. We conclude: Allergy to plant food in Granada is a more severe phenotype than other geographic areas from Spain, especially in young adults sensitized by different LTPs and pollinosis to olive pollen.


La alergia a frutas y verduras es la alergia alimentaria más común en los países mediterráneos, especialmente el síndrome LTP (lipid transfer proteins). Este es el primer estudio que analiza múltiples características clínicas, alergológicas y terapéuticas de la alergia a vegetales en el sureste de España y que evalúa si difieren en urgencias o consultas externas. Se trata de un estudio prospectivo de pacientes que consultan por primera vez debido a alergia a verduras en Granada, España. Registramos datos demográficos, clínicos, alergológicos y tratamiento prescrito. Las características obtenidas concuerdan con las infomadas en la bibliografía actual, excepto en la mayor prevalencia de Pru p 3, Cor a 9 y Ara h 9, que predisponen a reacciones alérgicas graves. La conclusión es que la alergia a vegetales en Granada es un fenotipo más grave que en otras áreas de España, especialmente en adultos jóvenes sensibilizados a diferentes proteínas de transferencia de lípidos y polinosis a polen del olivo.

Introduction: Eosinophilic esophagitis (EoE) is a chronic esophageal atopic disease because sensitization to aeroallergens and foods allergens is very common. Objectives: This study is the first work that studies multiple characteristics of EoE in the southeast of Spain to know whether EoE in the patients of this region is similar to that of other regions of Spain in terms of demography, symptoms, and atopic characteristics. Method: It is an observational prospective study of patients diagnosed with EoE at Granada (Spain). We recorded demographic data (age, sex, and personal history of atopy), clinical data (impaction and dysphagia), allergologic data (prick test and specific immunoglobulin E) to foods, aeroallergen, and pan-allergens, and other endoscopic–histologic data and comorbidities. Results: The demographic, allergologic, and endoscopic characteristics of patients with EoE in Granada were similar to the rest of Spain, except the higher frequency of sensitization to olive pollen, food allergy, and anaphylaxis reactions. Conclusions: The higher frequency of sensitization to olive pollen and food allergy with severe clinical manifestations (anaphylaxis) in patients with EoE in Granada could have a negative impact on patients’ quality of life (AU)

BACKGROUND: Acrylates are plastic materials that are formed by the polymerization of monomers that are derived from acrylic or methacrylic acid; they have a wide range of applications, and they are increasingly used in medicine. CASE REPORT: We are reporting two clinical cases of contact dermatitis in the hospital environment; the first one is about a nurse (occupational exposure) and, the second one, about a patient who experienced contact dermatitis after a diagnostic procedure was performed. CONCLUSION: New sources of acrylates are identified every year due to the multiple uses of these synthetic resins; these materials are included in medical devices, both for orthopedic diagnosis or follow-up (telemetry, electrodes of electroencephalographs) and treatment (bone cement), as well as for surgery.


Antecedentes: Los acrilatos son materiales plásticos que se forman por la polimerización de monómeros derivados del ácido acrílico o metacrílico; tienen una amplia gama de aplicaciones y cada vez son más usados en el campo de la medicina. Casos clínicos: Presentamos dos casos clínicos de dermatitis de contacto en el entorno hospitalario: el primero se trata de una enfermera (exposición ocupacional) y el segundo, de un paciente a quien se le realizó un procedimiento diagnóstico. Conclusión: Cada año se identifican nuevas fuentes de acrilatos debido a las múltiples utilidades de estas resinas sintéticas; estos materiales se incluyen en dispositivos médicos, tanto para el diagnóstico o seguimiento (telemetría, electrodos de electroencefalógrafos) como para el tratamiento (cemento óseo) y cirugía ortopédica.

Humans evolved by losing the capacity to synthesize the glycan Galα1-3Galß1-(3)4GlcNAc-R (α-Gal), which resulted in the capacity to develop a protective response mediated by anti-α-Gal IgM/IgG antibodies against pathogens containing this modification on membrane proteins. As an evolutionary trade-off, humans can develop the alpha-Gal syndrome (AGS), a recently diagnosed disease mainly associated with allergic reactions to mammalian meat consumption. The etiology of the AGS is the exposure to tick bites and the IgE antibody response against α-Gal-containing glycoproteins and glycolipids. The objective of this study was to characterize the anti-α-Gal antibody response in association with the immune-mediated peripheral neuropathy, Guillain-Barré syndrome (GBS), and compare it with different factors known to modulate the antibody response to α-Gal such as exposure to tick bites and development of allergic reactions in response to tick bites. The results showed a significant decrease in the IgM/IgG response to α-Gal in GBS patients when compared to healthy individuals. In contrast, the IgM/IgG levels to α-Gal did not change in patients with allergic reactions to tick bites. The IgE response was not affected in GBS patients, but as expected, the IgE levels significantly increased in individuals exposed to tick bites and patients with tick-associated allergies. These results suggest that the immune pathways of anti-α-Gal IgM/IgG and IgE production are independent. Further studies should consider the susceptibility to allergic reactions to tick bites in GBS patients.

BACKGROUND: In allergology, the intradermal approach is generally used to establish an aetiological diagnosis, with limited experience in specific allergen immunotherapy. OBJECTIVE: To evaluate the efficacy and safety of immunotherapy with an allergen extract of glutaraldehyde-polymerized Phleum pratense, administered intradermally, in patients with rhinoconjunctivitis sensitized to grass pollen. METHODS: Multicentre, randomized, double-blind, placebo-controlled clinical trial in patients from 12 to 65 years of age with rhinitis or rhinoconjunctivitis, with or without asthma, due to grass pollen allergy. Patients were divided into three groups and received a total of six doses in a weekly interval, of either placebo; 0.03 or 0.06 µg of protein per dose of P pratense allergoid. The primary objective was to evaluate the combined symptoms and medication consumption score (CSMS). The secondary objectives were symptoms and medication, tolerance to the conjunctival provocation test, specific IgE and IgG4 antibodies and the safety profile according to the WAO scale. RESULTS: The dose of 0.06 µg of protein proved to be effective versus the placebo by significantly reducing CSMS and increasing tolerance to the allergenic extract in the conjunctival provocation test, after the first pollen season. This group showed a significant reduction in specific IgE after the second pollen season relative to the baseline. There were no variations in IgG4 levels. Only one grade 2 systemic reaction was recorded. CONCLUSION & CLINICAL RELEVANCE: Intradermal immunotherapy with P pratense allergoid has been shown to be effective and safe, reducing CSMS, increasing tolerance to the conjunctival provocation test and reducing IgE levels.
